OVERVIEW OF NURSE EDUCATION IN DISCHARGE PLANNING AND COMPLIANCE OF DIABETES MELLITUS PATIENT CONTROL POST-HOSPITALIZATION AT GRHA KEDOYA HOSPITAL, WEST JAKARTA IN 2023

Background: Diabetes Mellitus is a chronic metabolic disorder caused by impaired insulin production by the pancreas. Proper management, including health education by nurses in discharge planning, is essential to improving patient adherence to post-hospitalization follow-up. The provided education is expected to assist patients in self-care and prevent complications. Research Objective: This study aims to describe the impact of nurse education in discharge planning on post-hospitalization follow-up adherence among Diabetes Mellitus patients at Grha Kedoya Hospital, West Jakarta, in 2023. Research Methodology: This study employs the Quota Sampling method with Chi-square analysis to examine the relationship between nurse education in discharge planning and patient adherence to post-hospitalization follow-up. Research Findings: The majority of respondents were aged 46–55 years (42.3%) and male (57.6%). Before receiving education, 71.2% had suboptimal understanding, and only 57.6% adhered to post-hospitalization follow-up. After receiving education, understanding increased to 84.7%, and adherence rose to 76.3%. The Chi-square test results indicate a significant relationship between nurse education in discharge planning and improved post-hospitalization follow-up adherence. Conclusion and Recommendation: Education in discharge planning plays a crucial role in enhancing patient adherence to health monitoring. This study is expected to serve as a reference for nurses in improving their educational skills to encourage patients to maintain regular health check-ups
